At SIVEM® Pharmaceuticals ULC, a wholly owned subsidiary of McKesson Canada Corporation (“SIVEM®”, “we” or “us”), we value the trust that you place in us to appropriately use and protect your personal information. This Privacy Notice informs individuals who contact us about our products and visitors and regular users of the SIVEM® website (collectively referred to as the “Services”) about the purposes for which we may collect, use, and share personal information. Our privacy philosophy is based on the 10 fair information principles found in the Personal Information Protection and Electronic Documents Act (PIPEDA). We want you to be clear on how we process personal information and the ways in which we protect it.

Consent & Revocation of Consent
By submitting information to us in connection with the Services or using the Services you are providing your consent to the collection, use and disclosure of your personal information as set out in this Privacy Notice and we, as well as our staff, may rely on that consent. We may also be entitled by law to contact you for commercial purposes, subject to your right to withdraw consent detailed below, and for regulatory purposes.
Consent may be withdrawn at any time, subject to legal, regulatory, or contractual restrictions and reasonable notice; however, without such consent, we may not be able to provide you with the Services.
Commercial Electronic Communications: You can always limit the communications that we send to you. You may choose to stop receiving our newsletter or marketing emails, if applicable, by following the unsubscribe instructions included in these emails, contacting us using the contact information listed in the Contact section of this Privacy Notice below, or updating your account preferences through the Services. Please note that even if you opt-out of promotional communications, we may still need to contact you with important information about your account, and content made available or displayed to you via the Services will remain customized for you.
Health information: You can always modify the health information you make available to us by contacting the patient support services program administrator, using the contact information available in the enrollment form accessible through the SIVEM® website.
Personal Information We Collect
When you use the Services, we may collect personal information about you. The personal information we collect includes:
- Contact and demographic information, such as your name, gender, date of birth, contact information (postal code, city and province/territory, email address, physical address, telephone number).
- Technical information, such as log and usage data when you access or use our Services. This data includes service-related, diagnostic, usage, and performance information, which we record in log files. Depending on your interaction with us, this log data may contain your IP address, device information, browser type and settings, details about your activity on our Services (such as timestamps, viewed pages and files, searches, and feature usage), as well as device event information such as system activity, error reports and hardware settings.
- Health information, which may include information about your health conditions, medication history, and underlying medical conditions.
- Where applicable, healthcare providers may choose to provide information, such as first and last name, license number, information relating to their specialties and professional affiliations, and contact information (postal code, city and province/territory, email address, physical address, telephone number).
- Adverse event information, such as details of the reaction you suffered including the product causing the reaction, your use of it, other medicines you are/were taking at the time of the reaction, treatment you received for that reaction and other medical history considered relevant to manage and process adverse event, medical inquiry or quality complaint reports on our products. Contact information, questions asked about the event and the relationship between the reporter and the subject of the report or request, if not the same.
- Location information, which can be precise or imprecise, depending on how you interact with our Services and your device’s settings. This data includes information about your current location, obtained through GPS and other technologies based on your IP address. You have the option to refuse access or disable the location setting on your device to opt out. However, please note that opting out may restrict your use of certain Services.
- Where applicable, insurance information for the provision of health care, or health care related services such as a patient support program.
- Voice recording information for individuals who contact us and whom we contact for quality assurance and training purposes.
How Your Personal Information Is Collected
We may collect personal information about you in different ways when you use the Services:
- From you directly, for instance through your use of the Services; your interactions with us (for example, when you interact with our Services, newsletter, registrations, applications and surveys); or your purchase of products from us or participating retailers. You may also provide us with personal information through other means (for instance, verbally).
- Through your online or electronic activity when using the Services, including through the use of technologies, such as cookies, web beacons, data management and advertising platforms, and self-reported information and communications with us through or with respect to the Services. For example, your IP address is reported by your web browser whenever you visit a page on the internet. Whenever a user visits our Services, their IP address, along with the time of the visit and the pages accessed, is automatically recorded in our server log files.
- In some cases, information will be collected from sources other than the individual, for example third parties who represent they have the right to disclose such information to us such as your healthcare providers.
- From your healthcare provider(s), a medical records system, or pharmacy management or dispensation software. In such case, your personal health information such as purchases of medication may be collected or generated by your pharmacist or your healthcare provider(s).
- From you directly, a third party or from your health care provider(s), whomever is the reporter of an adverse event, a medical inquiry, or a quality complaint for the purposes identified herein.
How Your Personal Information is Used
We use your personal information with your consent to:
- Service Delivery and Management: Operate our business, including processing, completing and fulfilling requested transactions, and facilitate the provision and management of the Services.
- Website and Content Optimization: We analyze engagement with our website and communications to improve content, optimize performance, and tailor our website and communications. We may combine information about you from affiliates, subsidiaries, business partners, ad networks, other third parties, and from others to improve your experience with our products and services.
- Communication and Customer Support: When you contact us, for example, through email, telephone or through submitting a customer service inquiry, we may collect the information that you provide to us in the course of the communication to help respond to you, for purposes related to your communication, and to follow up with further information or requested materials about our products or services.
- Research and Development: We conduct surveys and market research to better understand and improve our products and services. Participation in these surveys or contests is completely voluntary and you have a choice whether or not to disclose your information. We may use a third-party service provider to conduct these surveys or contests. We will not share the personal information you provide through a contest or survey with other third parties without your prior consent. Your personal information may be used in a de-identified, aggregated, or anonymized form for data assessments and analytics, improving services, and generating reports for commercial or market research.
- IT and Service Management: Manage the use of our Services, and our information technology applications and systems, which may include using cookies and other similar technologies and investigating incidents. For example, we collect IP addresses for determining Services usage levels, identifying, and resolving server issues. We may ask you if you want to give our website access to your mobile device’s location. If you grant access to your mobile location, we may collect location data from your device for analytics and to better provide our Services to you.
- Marketing Communications: If you opt-in to receive marketing communications from us, we will keep you up to date on our products and services. You may withdraw your consent to receiving marketing communications at any time by following the opt-out instructions in each communication, or by contacting us as indicated in the Contact section below.
- Advertisements: Advertisements may be delivered by us or one or more third-party web advertisers. These third-party web advertisers may set cookies to allow the advertisement server operated by that third party to recognize your computer each time they send you an online advertisement. Accordingly, advertisement servers may compile information about where or whether you viewed their advertisements and which advertisements you clicked on. We may analyze your engagement with our or third-party products and services to understand the effectiveness of our marketing campaigns and to deliver customized products and services. - Product Safety and Reporting: To manage and process adverse event reports on our products, to respond to medical inquiry requests, or product and quality complaints. We only process necessary information to document to meet legal requirements for pharmacovigilance, product safety and quality, and inquiries. We may contact you for further information about the adverse event, quality complaint or medical inquiry reported. We consolidate this information to analyze product safety and provide mandatory reports to regulatory authorities. If we publish information regarding adverse events, such as case studies or summaries, we will take the necessary precautions to safeguard the privacy and anonymity of individuals involved, for example, by aggregating, de-identifying or anonymizing the information.
Cookies, Web Beacons, and Related Technologies
When you access the Services, we may collect certain information using our or third-party technologies such as cookies, web beacons and other similar technologies. Cookies are alphanumeric identifiers that are saved on your computer’s hard drive to enable our systems to recognize your browser between visits and remember, for example, your IP address, log in credentials or products you last searched on our website. Web beacons, also known as tracking pixels or pixel tags, are small image files embedded in a website or email that collect information about users’ online activity, such as pageviews, clicks, and interactions with our emails or ads.
For the purposes of this Privacy Notice, cookies, web beacons, and all other similar technologies are collectively referred to as “cookies”. We use cookies to:
- remember information you gave us so that you don't have to re-enter the information each time you use our Services;
- improve your online experience as well as to provide you with a better personalized advertisements on our Services and on third party websites;

Some SIVEM® products and services may allow you to directly access, correct, update, or delete your personal information through your account settings.
